In order to be complete, a filing shall contain the\n\nfollowing:\n\n1. A memorandum briefly summarizing the gist of the filing;\n\n2. An index to the filing;\n\n3. A clear and concise statement of the action desired to be\n\ntaken by the Commissioner;\n\n4. References to the sections of law and to rules and\n\nregulations which authorize the action desired to be taken by the\n\nInsurance Commissioner or which support the information contained in\n\nthe filing;\n\n5. An explanation of the application of the filing factors,\n\nwhich are contained in subsection A of Section 902.2 or subsection B\n\nof Section 985 of this title, together with assumptions and\n\nconclusions concerning such factors;\n\n6. References to exhibits and other documents contained in the\n\nfiling which are relied upon to support the action requested by the\n\nfiling; and\n\n7. Any other information required by the Commissioner.\n\nB. If the filer is an advisory organization or joint\n\nunderwriting association, it is sufficient for such information to\n\nbe provided in summary form for all the filer's members and\n\nsubscribers.\n\nC.
