If an insurer writing personal insurance uses credit\n\ninformation in underwriting or rating a consumer, the insurer or its\n\nagent shall disclose, either on the insurance application or at the\n\ntime the insurance application is taken, that it may obtain credit\n\ninformation in connection with such application. Such disclosure\n\nshall be either written or provided to an applicant in the same\n\nmedium as the application for insurance. The insurer need not\n\nprovide the disclosure statement required under this section to any\n\ninsured on a renewal policy, if such consumer has previously been\n\nprovided a disclosure statement.\n\nB. Use of the following example disclosure statement\n\nconstitutes compliance with this section: “In connection with this\n\napplication for insurance, we may review your credit report or\n\nobtain or use a credit-based insurance score based on the\n\ninformation contained in that credit report.
