Stat. tit. 37A, § 37A-2-144","heading":"Information to be furnished by limited liability","body":"company applicants.\n\nA. Any limited liability company, formed as provided for in the\n\nLimited Liability Company Act, may apply for a mixed beverage, beer\n\nand wine, bottle club, caterer, public event, beer distributor,\n\nretail wine or retail beer license issued pursuant to the Oklahoma\n\nAlcoholic Beverage Control Act. Any limited liability company\n\napplying for a license shall submit to the ABLE Commission, the\n\nfollowing:\n\n1. A Certificate of Good Standing from the Office of the\n\nSecretary of State;\n\n2. The Articles of Organization with all amendments and\n\ncorrections filed with the Office of the Secretary of State with\n\nproof that same has been filed in accordance with the Limited\n\nLiability Company Act;\n\n3. The name and address of the resident agent;\n\n4. The name and address of the manager;\n\n5. The operating agreement;\n\n6. A current list of the full name, social security number and\n\naddress of each member; and\n\n7. A copy of the issued Certificate of Membership Interest for\n\neach member.\n\nB. A limited liability company licensee shall notify the ABLE\n\nCommission in writing of any change in the manager of the licensed\n\ncompany within thirty (30) days of the change and shall pay a fee of\n\nOne Hundred Dollars ($100.00) for each notification of change.\n\nC. A limited liability company shall notify the ABLE Commission\n\nin writing any time a membership is assigned or members are added or\n\ndisassociated within thirty (30) days of the change. The limited\n\nliability company shall pay a fee of One Hundred Dollars ($100.00)\n\nfor each notification of change.\n\nD. The ABLE Commission may disapprove a change of manager or\n\nnew membership in a licensed liability company if the ABLE\n\nCommission feels that such change would materially affect any\n\nconditions under which the license was issued, such that the license\n\nwould not have been issued had such change been in existence at the\n\ntime of the original application. If such disapproval occurs, the\n\nABLE Commission shall notify the licensee in writing and allow a\n\nreasonable time for the licensee to remove such manager or for a\n\nmember to be disassociated from the company; provided, a reasonable\n\ntime not exceed a ninety-day period following notification of denial\n\nby the ABLE Commission. Failure to comply with the provisions of\n\nthis subsection may result in revocation or suspension of such\n\nlicense.\n\nE. Any person who has been a licensee, a partner in a license,\n\nan officer, director or fifteen percent (15%) or more stockholder of\n\na corporation holding a license revoked or suspended, pursuant to\n\nthe provisions of the Oklahoma Alcoholic Beverage Control Act, shall\n\nnot serve as a manager or be a member in a limited liability company\n\nseeking a license pursuant to the provisions of the Oklahoma\n\nAlcoholic Beverage Control Act for a period of twelve (12) months\n\nfrom the date the license was revoked or suspended.\n\nF. Any person who has been a manager, member or participant in\n\nany business entity which was a manager or member of a limited\n\nliability company which has been denied a license or has a license\n\nrevoked or suspended, pursuant to the provisions of the Oklahoma\n\nAlcoholic Beverage Control Act shall not serve as a manager or\n\nmember in a limited liability company seeking a license pursuant to\n\nthe provisions of the Oklahoma Alcoholic Beverage Control Act for a\n\nperiod of twelve (12) months from date the license was revoked or\n\nsuspended.\n\nG.
