Each member of the general jury panel, when reporting for\n\nduty as provided for in Section 20 of this title, shall take and\n\nsubscribe to an oath or affirmation which shall be in substantially\n\nthe following form:\n\nOATH\n\nI, the undersigned, do solemnly swear or affirm that I am a\n\ncitizen of the United States and a resident of the State of\n\nOklahoma, County of __________. I further swear or affirm that I am\n\neighteen years of age or older.\n\nI further swear or affirm that I have not been convicted of a\n\nfelony for which a period of time equal to the original judgment and\n\nsentence has not expired, or for which I have not been pardoned. I\n\nfurther swear or affirm that I am not now adjudicated as being\n\nmentally incompetent and that I do not otherwise have a mental\n\ncondition which makes me incapable of performing jury service.\n\nI state under penalty of perjury under the laws of Oklahoma that\n\nthe foregoing is true and correct.\n\n________________________________________________ ________________\n\n(Signature or mark of general jury panel member) (Date and place)\n\nB. The oath or affirmation provided for in subsection A of this\n\nsection shall be maintained in the office of the court clerk as a\n\njudicial record.
