The Administrative Director of the Courts shall develop a\n\nstandard form to be used as a summons for service on the grand and\n\npetit juries in the district courts of this state. The standard\n\nform shall include the time, place and the name of the court where\n\nsaid jurors are required to attend.\n\nB. The summons shall be served by mailing a copy of such\n\nsummons to the person selected for service not less than ten (10)\n\ndays before the day said person is to appear as a juror in such\n\ncourt. The person mailing the summons shall make a return of such\n\nservice by filing an affidavit stating the date of mailing the\n\nsummons; provided, that this shall not prevent service of special\n\nvenire or talesman by the sheriff of the county.\n\nC. In any district court where an electronic jury management\n\nsystem is implemented pursuant to Section 13 of this act, grand and\n\npetit jurors may be summoned by using the automated process of\n\ncreating and mailing juror summons provided in the electronic jury\n\nmanagement system. The Administrative Director of the Courts shall\n\ndevelop a standard juror summons form to be used in the system,\n\nwhich shall include a section for the district court to add\n\ninformation specific to the local court.
