Any person desiring to conduct a race meeting may apply to\n\nthe Commission for an organization license. The application shall\n\nbe made on a form prescribed and furnished by the Commission and\n\nshall include a search waiver. The application shall contain the\n\nfollowing information:\n\n1. the dates on which the applicant intends to conduct the race\n\nmeeting; and\n\n2. the hours of each racing day between which the applicant\n\nintends to conduct horse racing at such meeting; and\n\n3. the location where the applicant proposes to conduct the\n\nrace meeting; and\n\n4. the name and mailing address of the person, association, or\n\ncorporation making the application; and\n\n5. if the applicant is a corporation:\n\na. a certified copy of the articles of incorporation and\n\nbylaws, and\n\nb. the names and mailing addresses of all stockholders\n\nwho own at least three percent (3%) of the total stock\n\nissued by the corporation, officers, and directors and\n\nthe number of shares of stock owned by each; and\n\n6. if the applicant is a partnership:\n\na. a copy of the partnership agreement, and\n\nb. the names and mailing addresses of all general and\n\nlimited partners with a statement of their respective\n\ninterest in the partnership; and\n\n7. any other information the Commission may require.\n\nB. A separate application shall be filed for each race meeting\n\nwhich such person proposes to conduct. The application:\n\n1. if made by an individual, shall be signed and verified under\n\noath by the individual; and\n\n2. if made by more than one individual or by a partnership,\n\nshall be signed and verified under oath by at least two of the\n\nindividuals or members of the partnership; and\n\n3. if made by an association, a corporation, or any other\n\nentity, shall be signed by the president, attested to by the\n\nsecretary under the seal of such association or corporation if it\n\nhas a seal, and verified under oath by one of the signing officers.\n\nC. No person shall own any silent or undisclosed interest in\n\nany entity requesting an organization license.\n\nD.
