Any organization desiring to conduct a charity game in this\n\nstate shall apply to the ABLE Commission for an organization\n\nlicense. An organization license shall only be issued to an\n\norganization and shall be used by that organization only. A\n\nbusiness entity shall not be qualified under any conditions to hold\n\nor use an organization license.\n\nB. An organization shall be limited to only one license and to\n\ndoing business at only one location.\n\nC. An organization shall use only disposable bingo faces\n\npurchased from a licensed distributor. Only one game shall be\n\nplayed on each bingo face; provided, nothing in this section shall\n\nbe construed to exclude progressive games.\n\nD. Only an organization or an employee or manager of an\n\norganization shall conduct a charity game for which a charge is\n\nmade. Compensation may be paid to an organization or other person\n\nfor conducting a charity game. Compensation for conducting charity\n\ngames shall be paid in accordance with minimum wage provisions of\n\nfederal law, and shall not exceed two times the amount of the\n\nminimum wage specified pursuant to federal law.\n\nE. An initial application for an organization license shall be\n\nsworn and attested to by a principal officer of the applicant\n\norganization and shall include:\n\n1. A certified copy of the document from the U.S. Internal\n\nRevenue Service which grants the applicant tax-exempt status and the\n\nfederal identification number;\n\n2. A certified copy of the articles of incorporation and\n\ncertificate of incorporation of the organization;\n\n3. A copy of the bylaws of the organization;\n\n4. A copy of the minutes of the meeting of the organization at\n\nwhich the governing body was elected and the terms of office of each\n\nmember of the governing body;\n\n5. A copy of the minutes of the meeting of the governing body\n\nof the organization at which the application for the license was\n\nauthorized;\n\n6. The name and address of a person authorized to receive\n\nservice of process on behalf of the organization;\n\n7. The address of the location where the charity games will be\n\nconducted; and\n\n8. Such other information deemed necessary by the Commission to\n\nassure eligibility for a license.\n\nF. A renewal application shall only include any changes in the\n\ninformation required to be submitted with the initial application.\n\nG.
