Any person or business entity desiring to sell or supply any\n\ncharity game equipment to a licensed organization in this state\n\nshall apply to the ABLE Commission for a distributor license.\n\nB. An application for a distributor license shall include:\n\n1. The name and address of the applicant and the name and\n\naddress of each of its separate locations distributing charity game\n\nequipment;\n\n2. The name and address of all owners of the distributing\n\nbusiness entity, if the business entity is not a corporation. If\n\nthe business entity is a corporation, the name and address of each\n\nof the officers and directors of the corporation and of each\n\nstockholder owning ten percent (10%) or more of any class of stock\n\nin the corporation; and\n\n3.
