Stat. tit. 4, § 4-499.4","heading":"Rules - Sterilization agreement","body":"Releasing agencies may adopt any additional rules to implement\n\nthe Dog and Cat Sterilization Act, provided said rules do not\n\nconflict with the provisions or purpose of the Dog and Cat\n\nSterilization Act to require the spaying and neutering of all dogs\n\nand cats adopted from releasing agencies. The sterilization\n\nagreement to be used by releasing agencies shall be in substantially\n\nthe following form:\n\nSTERILIZATION AGREEMENT\n\nThis Agreement is made and entered into this ____ day of\n\n__________, 19__, by and between:\n\n(Releasing Agency) (New Owner)\n\n__________________________ ______________________________\n\nName Name\n\n__________________________ ______________________________\n\nAddress Address\n\n__________________________ ______________________________\n\nCity Telephone City Telephone\n\nIn consideration of the releasing of said animal, and in further\n\nconsideration of mutual obligations herein, Releasing Agency\n\nreleases the following animal to the New Owner:\n\n_________________________________________________________\n\n(describe animal)\n\n1. Releasing Agency agrees to release the above listed animal\n\ninto the care of the New Owner and refund the New Owner's\n\nspay/neuter deposit provided that:\n\n(1) The animal is sterilized by a graduate licensed\n\nveterinarian by ______________________\n\n(give date)\n\n(2) A written statement signed by the veterinarian\n\nperforming the sterilization, that the animal has\n\nbeen sterilized by the stated date is given to the\n\nReleasing Agency.\n\n2.
