An unemployed individual must file an initial claim for\n\nunemployment benefits by completing the required forms through the\n\nInternet Claims service provided by the Commission, or by completing\n\nall forms necessary to process an initial claim in a local office of\n\nthe Commission or any alternate site designated by the Commission to\n\ntake unemployment benefit claims. The Commission may obtain\n\nadditional information regarding an individual's claim through any\n\nform of telecommunication, writing, or interview. An unemployed\n\nindividual must file a claim by telecommunication or by Internet\n\nutilizing the digital services portal to create an account to access\n\nbenefits with respect to each week in accordance with such rule as\n\nthe Commission may prescribe.\n\nB. With respect to each week, the claimant must provide the\n\nCommission with a true and correct statement of all material facts\n\nrelating to unemployment; ability to work; availability for work;\n\nactivities or conditions which could restrict the individual from\n\nseeking or immediately accepting full-time employment or part-time\n\nwork if subsection (4) of Section 2-408 of this title applies;\n\napplications for or receipt of workers' compensation benefits;\n\nemployment and earnings; and the reporting of other income from\n\nretirement, pension, disability, self-employment, education or\n\ntraining allowances.\n\nC. No claim will be allowed or paid unless the claimant resides\n\nwithin a state or foreign country with which the State of Oklahoma\n\nhas entered into a reciprocal or cooperative arrangement pursuant to\n\nSection 4-701 et seq. of this title.\n\nD. The Commission may require the individual to produce\n\ndocuments or information relevant to the claim for benefits. If the\n\nindividual fails to produce it, the individual's claim for\n\nunemployment benefits may be disqualified indefinitely by the\n\nCommission until the information is produced.
