Stat. tit. 40, § 40-80","heading":"School attendance certificate to be presented - Duplicate","body":"of schooling certificate - Blank form - Form of certificate.\n\nThe age and schooling certificate shall not be approved until\n\nthe parent or guardian of such child shall present a school\n\nattendance certificate as hereinafter prescribed. A duplicate of\n\nsuch age and schooling certificate shall be filled out and sent by\n\nthe school officer, before whom the same is made, to the\n\nCommissioner of Labor. The blank forms for school attendance\n\ncertificate and for the age and schooling certificate shall be\n\nsupplied to the principal, headmaster, or equivalent administrative\n\nofficer of the school or to one of the child's parents if the child\n\nis being schooled at home by the State Superintendent of Public\n\nInstruction as hereinafter indicated.\n\nSCHOOL ATTENDANCE CERTIFICATE.\n\nand write and that according to the records of this school and in my\n\nhas attended school during the full school term of the preceding\n\nyear.\n\nAGE AND SCHOOLING CERTIFICATE.\n\n(number of years and months old).\n\n(Signature of parent or guardian).\n\n(name of person signing), and made oath that the foregoing\n\ncertificate is true to the best of his (or her) knowledge and\n\nbelief.\n\nsufficient reason to doubt that he (or she) is of the age therein\n\ncertified.\n\nOWNER OF CERTIFICATE.\n\nis to be surrendered to him (or her) whenever he (or she) leaves the\n\nservice of the employer holding the same, but if not claimed by said\n\nchild within thirty days after leaving said service, shall be sent\n\nto the Commissioner of Labor.\n\n(Signature of officer, with name of city,\n\ntown or county, and date.)","path":["OK Code","Title 40"],"source_url":"https://www.oklegislature.gov/OK_Statutes/CompleteTitles/os40.pdf","current_through":"2026-08-14","vintage":"open-us-law v2026.08, retrieved 2026-09-14","retrieved_at":"2026-09-14T18:32:36Z","sha256":"58daba8fb3c6ae8cdd708ed0ddc43d0ca1438fdd4fdfc6f64200d29ce821e980","source_id":"us-ok","stale":false,"prev":"us-ok/okla.-stat.-tit.-40-40-79","next":"us-ok/okla.-stat.-tit.-40-40-800"},"notice":"GroundRules: Original legal text.
