Stat. tit. 43, § 43-424","heading":"Forms for petition and court order - Modification of","body":"visitation rights.\n\nA. The Office of the Court Administrator shall develop:\n\n1. A form for use in petitioning the court for inclusion in the\n\nchild visitation registry which shall be distributed to all court\n\nclerk offices; and\n\n2. A form for the court's order requiring participation in the\n\nregistry. This form shall provide for the following:\n\na. a requirement that a copy of the order be given to\n\neach parent, the child visitation registry agency, and\n\ncourt file,\n\nb. a determination of who is authorized to pick up or\n\ndeliver a child to the child visitation registry\n\nagency. The list may include, but is not limited to,\n\nparents, stepparents, and grandparents,\n\nc. a determination of when the participants shall meet to\n\npick up or deliver a child to the child visitation\n\nregistry agency. This decision shall include specific\n\ndays of the week and time periods,\n\nd. the date when participation in the program shall begin\n\nor end, and\n\ne. a requirement that the participant delivering the\n\nchild to the registry must wait at the agency and sign\n\nout after the participant picking up the child has\n\ndeparted from the agency.\n\nB.
