Persons desiring to be married in this state shall submit an\n\napplication in writing signed and sworn to in person before the\n\nclerk of the district court by both of the parties setting forth:\n\n1. The place of residence of each party;\n\n2. The full legal name and the age of each party as they appear\n\nupon or are calculable from a certified copy of the birth\n\ncertificate, the current driver license or identification card, the\n\ncurrent passport or visa, or any other certificate, license or\n\ndocument issued by or existing pursuant to the laws of any nation or\n\nof any state, or political subdivision thereof, accepted as proof of\n\nidentity and age;\n\n3. For each party, the full name by which the party will be\n\nknown after the marriage as provided in subsection E of this\n\nsection, which shall become the full legal name of the party upon\n\nthe filing of the marriage license and certificate with the court,\n\nas required by law; provided, however, a marriage certificate issued\n\nprior to June 8, 2006, or any marriage certificate which contains an\n\nerroneous or misspelled name of a party, shall be reissued upon\n\nrequest by the certificate holder to include the information\n\nrequired by this paragraph. Such reissued certificate shall reflect\n\nthe original marriage date and shall be signed by the court clerk\n\nwith a notation by the clerk that the certificate is \"reissued\" or\n\n\"amended\". Signatures of the officiant and original witnesses shall\n\nnot be required;\n\n4. That the parties are not disqualified from or incapable of\n\nentering into the marriage relation; and\n\n5. Whether the parties have successfully completed a premarital\n\ncounseling program.\n\nB. 1. Upon application pursuant to this section and the\n\npayment of fees as provided in Section 31 of Title 28 of the\n\nOklahoma Statutes, if the clerk of the district court is satisfied\n\nof the truth and sufficiency of the application and that there is no\n\nlegal impediment to such marriage, the court clerk shall issue the\n\nmarriage license authorizing the marriage and a marriage\n\ncertificate, which shall be incorporated as one document. As\n\nrequired by law, the marriage certificate shall be completed\n\nimmediately following the marriage, and the marriage license and\n\ncertificate shall be returned to the court clerk.\n\n2. Parties to be married and who present a certificate to the\n\nclerk of the district court that states the parties have completed\n\nthe premarital counseling program pursuant to Section 5.1 of this\n\ntitle shall be entitled to pay a reduced fee for a marriage license\n\nin an amount provided in Section 31 of Title 28 of the Oklahoma\n\nStatutes.\n\nC. In the event that one or both of the parties are under legal\n\nage, the application shall have been on file in the court clerk's\n\noffice for a period of not less than seventy-two (72) hours prior to\n\nissuance of the marriage license.\n\nD. The marriage license shall be valid in any county within the\n\nstate.\n\nE. On the marriage certificate, the full name by which a party\n\nshall be known after marriage may be any combination of the legal\n\nfirst, middle, and last names, given names and surnames, of either\n\nparty, including segments of names or initials, with or without\n\nhyphens. However, the marriage certificate shall not be used to\n\nchange the name of a party to an entirely different name which is\n\nnot derived from the legal name of either party. The party shall\n\npetition for a name change using the process established in Section\n\n1631 of Title 12 of the Oklahoma Statutes.\n\nF.
