Stat. tit. 43, § 43-571-106","heading":"Petition - Requirements","body":"A petition under this act shall be verified and include a copy\n\nof any existing child custody determination, if available. The\n\npetition shall specify the risk factors for abduction, including the\n\nrelevant factors described in Section 7 of this act. Subject to\n\nsubsection E of Section 551-209 of Title 43 of the Oklahoma\n\nStatutes, if reasonably ascertainable, the petition shall contain:\n\n1. The name, date of birth, and gender of the child;\n\n2. The customary address and current physical location of the\n\nchild;\n\n3. The identity, customary address, and current physical\n\nlocation of the respondent;\n\n4. A statement of whether a prior action to prevent abduction\n\nor domestic violence has been filed by a party or other individual\n\nor entity having custody of the child, and the date, location, and\n\ndisposition of the action;\n\n5. A statement of whether a party to the proceeding has been\n\narrested for a crime related to domestic violence, stalking, or\n\nchild abuse or neglect, and the date, location, and disposition of\n\nthe case; and\n\n6.
