Stat. tit. 43A, § 43A-4-102","heading":"Individualized treatment plans - Requirements","body":"There shall be developed during a person’s stay in a mental\n\nhealth facility, an individualized treatment plan which shall be\n\nspecifically tailored to such person’s treatment needs. Each plan\n\nshall clearly include the following:\n\n1. A statement of treatment goals or objectives, based upon and\n\nrelated to a proper evaluation, which can be reasonably achieved\n\nwithin a designated time interval;\n\n2. Treatment methods and procedures to be used to obtain these\n\ngoals, which methods and procedures are related to these goals and\n\nwhich include specific prognosis for achieving each of these goals;\n\n3. Identification of the types of professional personnel who\n\nshall carry out the treatment procedures, including appropriate\n\nmedical or other professional involvement by a physician or other\n\nhealth professional properly qualified to fulfill legal requirements\n\nmandated under state and federal law;\n\n4. Documentation of consumer involvement and, if applicable,\n\naccordance with the treatment plan by the consumer; and\n\n5.
