Stat. tit. 43A, § 43A-5-306","heading":"Certificate of physician","body":"The application described in Section 5-305 of this title shall\n\nbe accompanied by a certificate in duplicate signed by a licensed\n\ndoctor of medicine or osteopathic physician who is duly licensed to\n\npractice his such profession by the Oklahoma State Board of Medical\n\nLicensure and Supervision or the Oklahoma Board of Osteopathic\n\nExaminers, who is not related by blood or marriage to the person\n\nbeing examined, and who has no interest in the estate of the person\n\nbeing examined. This certificate shall include the following:\n\n1. A statement that a physician licensed in this state has\n\npersonally examined the person;\n\n2. A statement that such physician is not related by blood or\n\nmarriage to the person being examined and has no interest in the\n\nestate of the person being examined;\n\n3. A determination that the person has a mental illness that\n\nrequires inpatient admission;\n\n4. A statement that the person may not be held at the facility\n\nfor longer than one hundred twenty (120) hours or five (5) days,\n\nexcluding weekends and holidays, past the time when such person has\n\nrevoked consent to stay for treatment;\n\n5. Information on the benefits and side effects of the\n\ntreatment the person will receive in an inpatient setting;\n\n6. Certification that the person has made a knowing and willing\n\nconsent to voluntary inpatient treatment; and\n\n7.
