Stat. tit. 43A, § 43A-5-308","heading":"Order authorizing admission","body":"The order of the judge of the district court authorizing the\n\nadmission of an applicant as a voluntary consumer pursuant to the\n\nprovisions of the Mental Hospital Voluntary Admission Procedures Act\n\nshall contain the following findings:\n\n1. A physician licensed in this state personally examined the\n\nperson requesting inpatient admission for psychiatric care;\n\n2. Such physician certified that the person has a mental\n\ncondition that requires inpatient admission;\n\n3. The person knowingly and willingly consented to voluntary\n\ninpatient admission;\n\n4. The person fully understands the benefits, consequences,\n\nconditions, and side effects of inpatient admission and treatment,\n\nand agrees to them; and\n\n5.
