A physical evaluation;\n\n2. A mental evaluation;\n\n3. A social history;\n\n4. A study of the individual's family and community situation;\n\n5. A list of available forms of care and treatment which may\n\nserve as an alternative to admission to a hospital;\n\n6. Powers of attorney or advance health care directives, if\n\nany; and\n\n7.
