If an informal conference has been held, the Department\n\nshall issue and furnish the applicant and persons who are parties to\n\nthe administrative proceedings with the written finding of the\n\nDepartment, granting or denying the permit in whole or in part and\n\nstating the reasons therefor, within the sixty (60) days of the\n\nhearings in the informal conference.\n\nB. If no informal conference has been held, the Department\n\nshall notify the applicant for a permit within a reasonable time,\n\nwhether the application has been approved or disapproved in whole or\n\nin part.\n\nC. If the application is approved, the permit shall be issued.\n\nIf the application is disapproved, specific reasons shall be set\n\nforth in the notification. Within thirty (30) days after the\n\napplicant is notified of the final decision of the Department on the\n\npermit application, the applicant or any person with an interest\n\nwhich is or may be adversely affected may request a hearing on the\n\nreasons for the final determination. The Department shall hold a\n\nhearing within thirty (30) days of such request and provide\n\nnotification to all interested parties at the time that the\n\napplicant is so notified. Such hearing shall be of record,\n\nadjudicatory in nature and no person who presided at an informal\n\nconference shall either preside at the hearing or participate in\n\nthis decision or in any administrative appeal.
