Stat. tit. 45, § 45-778","heading":"Suspension or revocation of permit","body":"If the Department determines that a pattern of violations of any\n\nrequirements of this act exists or has existed, and if the\n\nDepartment also finds that such violations are caused by the\n\nunwarranted or willful failure of the permittee to comply with any\n\nrequirements of this act, the Department shall immediately issue an\n\norder to the permittee to show cause as to why the permit should not\n\nbe suspended or revoked and shall provide opportunity for a public\n\nhearing. If a hearing is requested the Department shall inform all\n\ninterested parties of the time and place of the hearing.
