Stat. tit. 45, § 45-913","heading":"Loading, hauling or dumping - Rules and procedures","body":"The following rules and procedures shall be complied with when\n\nloading, hauling or dumping:\n\n1. Equipment shall be inspected each shift by a qualified\n\nperson designated by the operator. Equipment defects affecting\n\nsafety shall be reported immediately;\n\n2. Equipment defects affecting safety shall be corrected before\n\nthe equipment is used;\n\n3. Powered mobile equipment shall be provided with adequate\n\nbrakes;\n\n4. Powered mobile haulage equipment shall be provided with\n\naudible warning devices. Lights shall be provided on both ends when\n\nrequired;\n\n5. Equipment operators shall be certain, by signal or other\n\nmeans, that all persons are in the clear before starting or moving\n\nequipment;\n\n6. When the entire length of a conveyor is visible from the\n\nstarting switch, the operator shall visually check to make certain\n\nthat all persons are in the clear before starting the conveyor. When\n\nthe entire length of the conveyor is not visible from the starting\n\nswitch, a positive audible warning system shall be installed and\n\noperated to warn persons that the conveyor will be started;\n\n7. Trucks, shuttle cars and front-end loaders shall be equipped\n\nwith emergency brakes separate and independent of the regular\n\nbraking system;\n\n8. Operators' cabs shall be constructed to permit operators to\n\nsee without difficulty and shall be reasonably comfortable;\n\n9. Cab windows shall be of safety glass or equivalent, in good\n\ncondition, and be kept clean;\n\n10. Cabs of mobile equipment shall be kept free of extraneous\n\nmaterials;\n\n11. Adequate back stops or brakes shall be installed on\n\ninclined conveyor drive units to prevent conveyors from running in\n\nreverse if a hazard to personnel will result;\n\n12. No person shall be permitted to ride a power-driven chain\n\nbelt or bucket conveyor unless the chain, belt or conveyor is\n\nspecifically designed for the transportation of persons;\n\n13. Equipment operating speeds shall be prudent and consistent\n\nwith conditions of roadway, grades, clearance, visibility, traffic\n\nand the type of equipment used;\n\n14. Dust control measures shall be taken where dust\n\nsignificantly reduces visibility of equipment operators. Haulage\n\nroads shall be wet down as necessary unless dust is controlled\n\nadequately by other methods;\n\n15. Mobile equipment operators shall have full control of the\n\nequipment while it is in motion;\n\n16. Dippers, buckets, loading booms or heavy suspended loads\n\nshall not be swung over the cabs of haulage vehicles until the\n\ndrivers are out of the cabs and in safe locations, unless the trucks\n\nare designed specifically to protect the drivers from falling\n\nmaterial;\n\n17. Only authorized persons shall be present in areas of\n\nloading or dumping operations;\n\n18. Unless safe provisions are made for persons to mount or\n\nleave equipment while it is in operation, the operator shall be\n\nnotified of their intentions before getting on or off;\n\n19. Persons shall not work or pass under the buckets or booms\n\nof loaders in operation;\n\n20. When traveling between work areas, the equipment shall be\n\nsecured in the travel position;\n\n21. Dippers, buckets, scraper blades, and similar movable parts\n\nshall be secured or lowered to the ground when not in use;\n\n22. Persons shall not ride in dippers, buckets, forks,\n\nclamshells or other parts of any equipment not specifically designed\n\nfor the transportation of persons;\n\n23. Loaded cars or trucks shall not be moved until the loads\n\nare trimmed properly;\n\n24. Electrically powered mobile equipment shall not be left\n\nunattended unless the master switch is in the \"off\" position, all\n\noperating controls are in the \"neutral\" position, and the brakes are\n\nset or other equivalent precautions are taken against rolling;\n\n25. Mobile equipment shall not be left unattended unless the\n\nbrakes are set. The wheels shall be turned into a bank or wall, or\nroperly;\n\n24. Electrically powered mobile equipment shall not be left\n\nunattended unless the master switch is in the \"off\" position, all\n\noperating controls are in the \"neutral\" position, and the brakes are\n\nset or other equivalent precautions are taken against rolling;\n\n25. Mobile equipment shall not be left unattended unless the\n\nbrakes are set. The wheels shall be turned into a bank or wall, or\n\nshall be blocked, when such equipment is parked on a grade;\n\n26. Persons shall not ride on top of loaded haulage equipment;\n\n27. Persons shall not ride outside the cabs and beds of mobile\n\nequipment;\n\n28. Equipment which is to be hauled shall be properly loaded\n\nand secured;\n\n29. Dumping locations and haulage roads shall be kept\n\nreasonably free of water, debris and spillage;\n\n30. Berms, bumper blocks, safety hooks or similar means shall\n\nbe provided to prevent overtravel and overturning at dumping\n\nlocations;\n\n31. If truck spotters are used, they shall be well in the clear\n\nwhile trucks are backing into dumping position and dumping; lights\n\nshall be used at night to direct trucks;\n\n32. Where overhead clearance is restricted, warning devices\n\nshall be installed and the restricted area shall be conspicuously\n\nmarked;\n\n33. Ramps and dumps shall be of solid construction, of ample\n\nwidth, have ample side clearance and headroom, and be kept\n\nreasonably free of spillage;\n\n34. Lights, flares or other warning devices shall be posted\n\nwhen parked equipment creates a hazard to vehicular traffic;\n\n35. Tires shall be deflated before repairs on them are started,\n\nand adequate means shall be provided to prevent wheel locking rims\n\nfrom creating a hazard during tire inflation;\n\n36. Any load extending more than four (4) feet beyond the rear\n\nof the vehicle body shall be marked clearly with a red flag by day\n\nand a red light by night;\n\n37. A tow bar shall be used to tow heavy equipment. A safety\n\nchain shall be used in conjunction with the tow bar; and\n\n38.
