As used in this section:\n\n1. \"Physician\" means any person holding a valid license to\n\npractice medicine and surgery, osteopathic medicine, chiropractic,\n\npodiatric medicine, or optometry, pursuant to the state licensing\n\nprovisions of Title 59 of the Oklahoma Statutes;\n\n2. \"Physician assistant\" means any person holding a valid\n\nlicense as a physician assistant, pursuant to the state licensing\n\nprovisions of the Physician Assistant Act;\n\n3. \"Advanced registered nurse practitioner\" means any person\n\nwho holds a current license as a registered nurse and a current\n\ncertificate of recognition for practice as an Advanced Registered\n\nNurse Practitioner as set forth in the Oklahoma Nursing Practice Act\n\npursuant to the state licensing provisions contained in paragraph 5\n\nof Section 567.3a of Title 59 of the Oklahoma Statutes; and\n\n4. \"Physical disability\" means an illness, disease, injury or\n\ncondition by reason of which a person:\n\na. cannot walk two hundred (200) feet without stopping to\n\nrest,\n\nb. cannot walk without the use of or assistance from a\n\nbrace, cane, crutch, another person, prosthetic\n\ndevice, wheelchair or other assistance device,\n\nc. is restricted to such an extent that the person's\n\nforced (respiratory) expiratory volume for one (1)\n\nsecond, when measured by spirometry, is less than one\n\n(1) liter, or the arterial oxygen tension is less than\n\nsixty (60) mm/hg on room air at rest,\n\nd. must use portable oxygen,\n\ne. has functional limitations which are classified in\n\nseverity as Class III or Class IV according to\n\nstandards set by the American Heart Association,\n\nf. is severely limited in the person's ability to walk\n\ndue to an arthritic, neurological or orthopedic\n\ncondition, or from complications due to pregnancy,\n\ng. is certified legally blind, or is missing one or more\n\nlimbs.\n\nB. Service Oklahoma shall issue a detachable placard indicating\n\nphysical disability to any person who submits an application on a\n\nform furnished by Service Oklahoma and certified by a physician,\n\nphysician assistant, or advanced registered nurse practitioner\n\nattesting that the applicant has a physical disability. The\n\nattestation of the physician, physician assistant, or advanced\n\nregistered nurse practitioner shall denote \"temporary\" as the type\n\nof placard requested and shall indicate an expiration date which the\n\nphysician, physician assistant, or advanced registered nurse\n\npractitioner estimates to be the date of termination of such\n\nphysical disability; however, if the physician, physician assistant,\n\nor advanced registered nurse practitioner certifies that the\n\nphysical disability is permanent, the physician, physician\n\nassistant, or advanced registered nurse practitioner shall denote\n\n\"five-year\" as the type of placard requested.\n\nC. Any placard issued by Service Oklahoma shall remain valid\n\nuntil:\n\n1. The placard expires;\n\n2. The person to whom the placard was issued requests a\n\nreplacement placard; or\n\n3. The placard is no longer needed by the person to whom the\n\nplacard was issued for the disability for which the placard was\n\noriginally issued, whereupon such placard shall be returned to\n\nService Oklahoma.\n\nD. 1. A five-year placard shall expire five (5) years from the\n\nlast day of the month in which the placard was issued. Upon the\n\nexpiration of a five-year placard, the person to whom such placard\n\nwas issued may obtain a subsequent placard by reapplying to Service\n\nOklahoma, in the same manner as provided in subsection B of this\n\nsection.\n\n2. A temporary placard shall indicate the expiration date which\n\nthe physician, physician assistant, or advanced registered nurse\n\npractitioner certifying the physical disability estimates to be the\n\ndate of termination of such physical disability, which shall not be\n\nlater than six (6) months from the date of issuance and upon which\n\ndate such placard shall expire and shall be returned to Service\n2. A temporary placard shall indicate the expiration date which\n\nthe physician, physician assistant, or advanced registered nurse\n\npractitioner certifying the physical disability estimates to be the\n\ndate of termination of such physical disability, which shall not be\n\nlater than six (6) months from the date of issuance and upon which\n\ndate such placard shall expire and shall be returned to Service\n\nOklahoma; provided, however, nothing in this paragraph shall be\n\nconstrued to prevent the holder from applying for another placard,\n\nas provided for in this section.\n\n3. In the event that a placard is lost or destroyed, the person\n\nto whom such placard was issued may apply in writing to Service\n\nOklahoma for a replacement placard, which Service Oklahoma shall\n\nissue with the same expiration date as the original placard.\n\n4. On and after January 1, 1998, all placards issued prior to\n\nOctober 31, 1990, shall expire on the last day of the month in which\n\nthe placard was issued, and the person to whom such placard was\n\nissued may follow the procedure provided for in subsection B of this\n\nsection to obtain a second or subsequent placard.\n\n5. On and after January 1, 2000, all placards issued between\n\nNovember 1, 1990, and June 30, 1995, shall expire on the last day of\n\nthe birth month of the person to whom the placard was issued, and\n\nthe person to whom such placard was issued may follow the procedure\n\nprovided for in subsection B of this section to obtain a second or\n\nsubsequent placard.\n\nE. A physician, physician assistant, or advanced registered\n\nnurse practitioner may sign an application certifying that a person\n\nhas a physical disability, as provided in subsection B of this\n\nsection, only if care and treatment of the illness, disease, injury\n\nor condition causing the physical disability of such person falls\n\nwithin the authorized scope of practice of the physician or\n\nphysician assistant, or advanced registered nurse practitioner.\n\nF. Service Oklahoma shall have the power to formulate, adopt\n\nand promulgate rules as may be necessary to implement and administer\n\nthe provisions of this section, including, but not limited to,\n\nprescribing the manner in which the placard is to be displayed on a\n\nmotor vehicle.\n\nG. All applications and information submitted in conjunction\n\nwith an application for a placard are confidential and Service\n\nOklahoma shall not be required to release to any person,\n\napplications, or placard information, in whole or in part and in any\n\nformat, except as otherwise provided for in this subsection.\n\nInformation contained on the application, including personally\n\nidentifiable information, shall only be released for the following\n\npurposes:\n\n1. For use by a law enforcement agency in carrying out its\n\nfunctions, specifically including parking enforcement; and\n\n2. For use in connection with any civil, criminal, or\n\nadministrative proceeding in any federal, state, or local court or\n\nagency.\n\nH. Nothing in this section shall prohibit the issuance of a\n\ntemporary placard to a pregnant woman whose condition, as determined\n\nby a physician, physician assistant, or advanced registered nurse\n\npractitioner, meets one or more of the categories described in\n\nparagraph 4 of subsection A of this section.","path":["OK Code","Title 47"],"source_url":"https://www.oklegislature.gov/OK_Statutes/CompleteTitles/os47.pdf","current_through":"2026-08-14","vintage":"open-us-law v2026.08, retrieved 2026-09-14","retrieved_at":"2026-09-14T18:32:36Z","sha256":"6c612764172b411aa66242d14eb7e280b91d01bb2dec762cc21bd2f3bbf0a122","source_id":"us-ok","stale":false,"prev":"us-ok/okla.-stat.-tit.-47-47-15-111","next":"us-ok/okla.-stat.-tit.-47-47-15-113"},"notice":"GroundRules: Original legal text.
