Every carrier, upon issuing an owner's policy, a renewal\n\nthereof, or a binder, shall supply a security verification form in\n\nduplicate to an owner for each insured vehicle on a form approved by\n\nthe Insurance Commissioner.\n\n1. The owner's security verification form shall contain the\n\nfollowing minimum information:\n\na. the name, address, and the five-digit National\n\nAssociation of Insurance Commissioners (NAIC) company\n\ncode of the carrier,\n\nb. the name, address, and telephone number of the agent\n\nor office where the existence of security may be\n\nverified, if other than the carrier,\n\nc. the name of the named insured; provided, the address\n\nof the named insured shall not be included,\n\nd. a notice that an owner's liability insurance policy\n\nhas been issued pursuant to the Compulsory Insurance\n\nLaw,\n\ne. the year of manufacture, make, model, and the vehicle\n\nidentification number of each insured motor vehicle,\n\nf. the inclusive dates the motor vehicle liability\n\ninsurance is in effect,\n\ng. the policy number,\n\nh. a warning to the owner that state law:\n\n(1) requires a current copy of the owner's security\n\nverification form must be surrendered to the\n\nmotor license agent or other registering agency\n\nupon application or renewal for a motor vehicle\n\nlicense plate,\n\n(2) requires the other copy of the owner's security\n\nverification form to be carried in the motor\n\nvehicle at all times, and produced by any driver\n\nof the vehicle upon request for inspection by any\n\npeace officer or representative of the Department\n\nof Public Safety. In case of an accident, the\n\nsecurity verification form shall be shown upon\n\nrequest of any person affected by the accident,\n\nand\n\ni. the statement: \"Examine policy exclusions carefully.\n\nThis form does not constitute any part of your\n\ninsurance policy.\"\n\n2. When a carrier issues an owner's policy providing blanket\n\nliability coverage for a fleet of motor vehicles, the requirement\n\nfor year of manufacture, make, model, and the vehicle identification\n\nnumber specified in subparagraph e of paragraph 1 of this subsection\n\nmay be omitted, provided the security verification form shall bear\n\nthe term \"Fleet Coverage\" and otherwise meet the provisions of the\n\nCompulsory Insurance Law.\n\n3. In the event the effective dates within an owner's policy\n\nexceed one (1) year, the carrier shall furnish the owner a copy of\n\nthe owner's security verification form at least annually in addition\n\nto the time of issuance or renewal in order for the owner to submit\n\nthe copy for motor vehicle registration purposes.\n\n4. In the event an owner's policy also provides liability\n\ncoverage which meets the requirements of an operator's policy, the\n\ncarrier may also issue to each person entitled thereto an operator's\n\nsecurity verification form as provided in this section.\n\nB. Every carrier, upon issuing an operator's policy, a renewal\n\nthereof, or a binder, may issue to the insured person a written\n\noperator's security verification form of a size which may\n\nconveniently be carried upon the person, containing the following\n\nminimum information:\n\n1. The name, address of the carrier, and the five-digit\n\nNational Association of Insurance Commissioners (NAIC) company code;\n\n2. The name, address, and telephone number of the person or\n\noffice where an inquiry may be made to verify the existence of\n\nsecurity;\n\n3. The name of the named insured; provided, the address of the\n\nnamed insured shall not be included;\n\n4. A notice that in accordance with the Compulsory Insurance\n\nLaw, liability coverage has been issued for the named insured;\n\n5. A statement reflecting the form may be carried in lieu of an\n\nowner's form pursuant to the Compulsory Insurance Law while\n\noperating a motor vehicle. The form shall be produced upon request\n\nof any peace officer or representative of the Department of Public\nbe included;\n\n4. A notice that in accordance with the Compulsory Insurance\n\nLaw, liability coverage has been issued for the named insured;\n\n5. A statement reflecting the form may be carried in lieu of an\n\nowner's form pursuant to the Compulsory Insurance Law while\n\noperating a motor vehicle. The form shall be produced upon request\n\nof any peace officer or representative of the Department of Public\n\nSafety. In case of an accident, the form shall be shown upon\n\nrequest of a person affected by an accident with a vehicle operated\n\nby the insured;\n\n6. The inclusive dates of liability coverage; and\n\n7. The policy number.\n\nC. A carrier may provide any additional information consistent\n\nwith the Compulsory Insurance Law of this state in an owner's or\n\noperator's security verification form but shall not be required to\n\nlist the actual amounts of liability coverage thereon. The security\n\nverification form shall not constitute nor be construed as any part\n\nof an insurance policy, renewal or binder.\n\nD. A carrier shall designate on the security verification form\n\nwhether the policy is a commercial auto policy.\n\nE.
