The name of the applicant, printed exactly as the applicant\n\nwill sign documents as a notary public;\n\n2. Former names of the applicant, if any;\n\n3. If a resident of this state, the county of residence and\n\nstreet address of the applicant;\n\n4. If a resident of another state, the county and street\n\naddress of the applicant's place of employment in Oklahoma and the\n\napplicant's residence address;\n\n5. Daytime phone number of the applicant;\n\n6. Electronic mail address of the applicant;\n\n7. A statement that the applicant is at least eighteen (18)\n\nyears of age;\n\n8. A designation of new, renewal or expired commission\n\nincluding an expiration date if applicable;\n\n9. A statement that the applicant is a citizen of the United\n\nStates;\n\n10. A statement that the applicant has never been convicted of\n\na felony;\n\n11. A statement that the applicant is able to read and write in\n\nEnglish; and\n\n12.
