The Oklahoma Health Care Authority shall develop a formal\n\nprogram for disease management to improve the quality of care and\n\nreduce the cost of care. The Authority is authorized to contract\n\nfor the development of the program if contracting is more cost\n\neffective to the state than developing the program internally. The\n\ndisease management program may utilize pharmacy services including,\n\nbut not limited to, Medication Management Therapy. The program may\n\ninclude, but not be limited to, asthma, diabetes, chronic\n\nobstructive pulmonary disease, renal disease and/or congestive heart\n\nfailure.\n\nThe disease management program shall consist of:\n\n1. Claims data analysis;\n\n2. Population selection and targeting;\n\n3. Intervention through educational tools for patients and\n\nproviders and treatment guidelines for physicians;\n\n4. Quality measurements of program structure, performance\n\nindicators, and outcome measures; and\n\n5. Reporting of outcome measure data.\n\nB.
