Contingent upon the availability of funding, for Medicaid\n\nclaims with dates of service on or after April 1, 2009, on all\n\npurchased wheeled mobility, the patient shall either have:\n\n1. A specialty evaluation that was performed by a licensed or\n\ncertified medical professional, such as a physical therapist,\n\noccupational therapist, or physician who has specific training and\n\nexperience in rehabilitation wheelchair evaluations, and that\n\ndocuments the medical necessity for the wheelchair and its special\n\nfeatures; or\n\n2. The wheelchair provided by a supplier that employs a RESNA-\n\ncertified assistive technology supplier or assistive technology\n\npractitioner who specializes in wheelchairs and who has direct, in-\n\nperson involvement in the wheelchair selection for the patient; and\n\n3. Met the requirements for medical necessity as determined and\n\napproved by the Oklahoma Health Care Authority.\n\nB.
