Gross inefficiency or incompetence;\n\n2. Violation of any provisions of the Waterworks and Wastewater\n\nWorks Operator Certification Act or applicable provisions of the\n\nOklahoma Environmental Quality Code, rules promulgated thereunder or\n\nthe terms of any certificate or order issued pursuant thereto; or\n\n3.
