Stat. tit. 59, § 59-1213","heading":"Applications for registration - Forms - Fees","body":"Applications for registration shall be made on forms prescribed\n\nand furnished by the Oklahoma Department of Agriculture, Food, and\n\nForestry, shall contain statements as to citizenship, residence, the\n\napplicant's education and a detailed summary of the applicant's\n\ntechnical work, and shall contain the names of not less than five\n\n(5) persons, of whom three (3) or more shall be forestry school\n\ngraduates, having personal or professional knowledge of the\n\napplicant's forestry experience. The forms shall also contain a\n\ncode of ethics prepared and approved by the Department essentially\n\nconforming to the code of ethics of the Society of American\n\nForesters. The registration fee for a certificate as a \"licensed\n\nforester\" shall be fixed by the Department but not to exceed Twenty-\n\nfive Dollars ($25.00), one-half (1/2) of which fee shall accompany\n\nthe application, the balance to be paid before issuance of the\n\ncertificate. Should the applicant fail or refuse to remit the\n\nremaining balance within thirty (30) days after being notified by\n\nmail that the applicant has successfully qualified, the applicant\n\nshall forfeit the right to have a certificate issued and the\n\napplicant may be required to again submit an original application\n\nand pay an original fee therefor.
