An application for a company license shall include:\n\n1. The address of the principal office of the applicant and the\n\naddress of each branch office of the applicant located within this\n\nstate;\n\n2. The name per business location under which the applicant\n\nintends to do business as a licensee;\n\n3. A statement explaining the extent and scope of the\n\napplicant's alarm, locksmith or fire sprinkler industry business;\n\n4. A photograph taken by the Department of Labor or an entity\n\napproved by the Department in accordance with the licensing\n\nprocedures adopted by the Department. If the applicant is a sole\n\nproprietor, the photo shall be of the applicant, or if the applicant\n\nis an entity, the photo shall be of each officer and of each partner\n\nor shareholder who owns an interest in the entity of twenty-five\n\npercent (25%) or greater;\n\n5. Two classifiable sets of fingerprints of the applicant, if\n\nthe applicant is a sole proprietor, or of each officer and of each\n\npartner or shareholder who owns a twenty-five percent (25%) or\n\ngreater interest in the applicant, if the applicant is an entity;\n\nand\n\n6. Such other information, statements or documents as may be\n\nrequired by the Commissioner of Labor.\n\nB. An applicant for an individual license shall provide such\n\ndocuments, statements or other information as may be required by the\n\nCommissioner, including two classifiable sets of fingerprints of the\n\napplicant. The fingerprints may be used for a national criminal\n\nhistory record check as defined by Section 150.9 of Title 74 of the\n\nOklahoma Statutes.\n\nC. Fees for license and renewal issued pursuant to the Alarm,\n\nLocksmith and Fire Sprinkler Industry Act shall be adopted by the\n\nDepartment of Labor. Provided, the fees provided for in this\n\nsubsection shall not exceed Two Hundred Fifty Dollars ($250.00). An\n\napplicant shall pay the license fee at the time the applicant makes\n\napplication.
