An application for a license shall include:\n\n1. The address of the principal office of the applicant and the\n\naddress of each branch office located within this state;\n\n2. The name of each business location under which the applicant\n\nintends to do business as a licensee;\n\n3. A statement as to the extent and scope of the fire\n\nextinguisher industry business of the applicant and all other\n\nbusinesses in which the applicant is engaged in this state;\n\n4. A recent photograph of the applicant of a type prescribed by\n\nthe State Fire Marshal Commission if the applicant is a sole\n\nproprietor, or a photograph of each officer and of each partner or\n\nshareholder who owns a twenty-five percent (25%) or greater interest\n\nin the applicant, if the applicant is an entity; and\n\n5. Such other information, statements, or documents as may be\n\nrequired by the State Fire Marshal Commission.\n\nB. An applicant for an individual license shall provide such\n\ndocuments, statements or other information as may be required by the\n\nState Fire Marshal Commission, including two classifiable sets of\n\nfingerprints of the applicant. The fingerprints may be used for a\n\nnational criminal history record check as defined by Section 150.9\n\nof Title 74 of the Oklahoma Statutes.\n\nC. Fees for license and license renewal issued pursuant to the\n\nFire Extinguisher Licensing Act shall be adopted by the State Fire\n\nMarshal Commission pursuant to Section 1820.19 of Title 59 of the\n\nOklahoma Statutes.
