No health spa shall offer or advertise health spa services\n\nunless first being registered with the Administrator of Consumer\n\nCredit. The registration shall:\n\n1. Disclose the address, ownership, date of first sales and\n\ndate of first opening of the health spa;\n\n2. State the name and address of the registered agent of the\n\nregistrant, if the registrant is a corporation;\n\n3. Be renewed each succeeding calendar year; and\n\n4. Be accompanied by an initial investigation and registration\n\nfee and an annual registration fee as prescribed by rule of the\n\nCommission on Consumer Credit.\n\nB. Each separate location where health spa services are offered\n\nshall be considered a separate health spa and shall file a separate\n\nregistration, even though the separate locations are owned or\n\noperated by the same owner.\n\nC. The Commission on Consumer Credit shall prescribe by rule a\n\nfee for each registration change, duplicate registration, or\n\nreturned check.\n\nD. The Commission on Consumer Credit shall prescribe by rule a\n\nlate fee for a registration not renewed on or before the expiration\n\ndate of the registration.\n\nE.
