Upon the receipt of an application and application fee,\n\nthe State Board of Examiners of Perfusionists may issue a\n\nprovisional license to practice perfusion for a period of one (1)\n\nyear to a person permitted, licensed or certified in another state,\n\nterritory, or possession of the United States who does not qualify\n\nfor a licensure by endorsement pursuant to Section 11 of this act\n\nbut has applied to take the examination and otherwise meets the\n\nqualifications of the Board. Provided, the applicant must show\n\nwritten evidence, verified by oath, that the applicant is currently\n\npracticing or has within the last six (6) months practiced perfusion\n\nin another state, territory, or possession of the United States.\n\n2. A graduate of a perfusion education program approved by the\n\nAccreditation Committee for Perfusion Education and approved by the\n\nCommittee on Allied Health Education and Accreditation of the\n\nAmerican Medical Association or their successors, who has applied to\n\ntake the examination and otherwise meets the qualifications of the\n\nBoard.\n\n3. A student currently enrolled in a perfusion education\n\nprogram approved by the Accreditation Committee for Perfusion\n\nEducation and approved by the Committee on Allied Health Education\n\nand Accreditation of the American Medical Association or their\n\nsuccessors may receive a provisional license as set out by the rules\n\nof the Board.\n\nB. A person to whom a provisional license is issued pursuant to\n\nthis section shall be under the supervision and direction of a\n\nlicensed perfusionist at all times during which the provisional\n\nlicensed perfusionist performs perfusion.
