The State Commissioner of Health shall, with the assistance\n\nof the Advisory Committee on Midwifery, establish qualifications for\n\nlicensure under Shepherd's Law.\n\nB. No person who is certified as, or holds himself or herself\n\nout to be, a Certified Professional Midwife or a Certified Midwife\n\nshall practice midwifery in this state without first applying for\n\nand obtaining a license from the State Commissioner of Health.\n\nC. Application shall be made to the Commissioner on a form\n\ncreated by the Department and posted on the website of the\n\nDepartment. The application shall be accompanied by a nonrefundable\n\napplication fee of One Thousand Dollars ($1,000.00) and such other\n\ninformation required by the Committee as established by rule. The\n\nlicense shall be valid for three (3) years from the date of\n\nissuance.\n\nD.
