Each licensee shall file an annual report with the\n\nAdministrator of the Department of Consumer Credit on the date of\n\nthe renewal application required in Section 10 of this act,\n\ncontaining the following information:\n\n1. The names and addresses of persons owning a controlling\n\ninterest in each licensee;\n\n2. The location of all places of business operated by the\n\nlicensee and the nature of the business conducted at each location;\n\n3. The names and addresses of all affiliated entities regulated\n\nunder Title 14A of the Oklahoma Statutes doing business in this\n\nstate;\n\n4. An audited financial statement, including, but not limited\n\nto, a balance sheet, statement of income or loss and statement of\n\nchanges in financial position, for the immediately preceding fiscal\n\nyear end, prepared in accordance with generally accepted accounting\n\nprinciples by a certified public accountant or public accounting\n\nfirm, neither of which is affiliated with the licensee; and\n\n5. If the licensee is a corporation, the names and addresses of\n\nits officers and directors; if the licensee is a partnership, the\n\nnames and addresses of the partners; or if the licensee is a limited\n\nliability company, the names and addresses of the board of governors\n\nor managers of the limited liability company.\n\nB. If the licensee holds two or more licenses or is affiliated\n\nwith other licensees, a composite report may be filed but shall not\n\nbe required.\n\nC. The reports shall be filed in a form that may reasonably be\n\nrequired by the Administrator and shall be sworn to by a responsible\n\nofficer of the licensee.\n\nD.
