All persons applying for licensure as an anesthesiologist\n\nassistant shall submit an application to the State Board of Medical\n\nLicensure and Supervision on forms approved by the Board.\n\nB. The application shall not be used for more than one (1) year\n\nfrom the date of original submission of the application and fee.\n\nAfter one (1) year from the date that the original application and\n\nfee have been received in the Board office, a new application and\n\nfee shall be required from any applicant who desires licensure as an\n\nanesthesiologist assistant.\n\nC.
