Any person applying for an oral maxillofacial surgery\n\nexpanded duty permit shall complete the requirements of this section\n\nfor submission and approval before the Board.\n\nB. The application shall be made to the Board in writing and\n\nshall be accompanied by the fee established by the Board, together\n\nwith the satisfactory proof that the applicant:\n\n1. Passes a background check with criteria established by the\n\nBoard; and\n\n2. Has completed all of the training requirements for the oral\n\nmaxillofacial surgery expanded duty permit as established by the\n\nBoard.\n\nC. An oral maxillofacial surgery expanded duty permit shall be\n\nconsidered a temporary training permit until all of the training\n\nrequirements, as established by the Board for the oral maxillofacial\n\nsurgery expanded duty permit, have been completed and approved by\n\nthe Board.\n\nD. A temporary training permit for the oral maxillofacial\n\nsurgery expanded duty shall not be extended beyond two (2) years.\n\nE. When performing the expanded duty of oral maxillofacial\n\nsurgery, dental assistants shall be under direct supervision or\n\ndirect visual supervision at all times by a licensed oral\n\nmaxillofacial surgeon, dentist, or pediatric dentist who holds a\n\nparenteral or general anesthesia permit; provided, only the oral\n\nmaxillofacial surgeon, dentist, or pediatric dentist, or an\n\nanesthesiologist or a Certified Registered Nurse Anesthetist, may\n\nadminister anesthesia and assess the patient’s level of sedation and\n\nonly the oral maxillofacial surgeon, dentist, or pediatric dentist\n\nmay perform surgery.\n\nF. An applicant for an oral maxillofacial surgery expanded duty\n\npermit shall provide satisfactory proof of:\n\n1. Successful completion of the Dental Anesthesia Assistant\n\nNational Certification Examination (DAANCE) provided by the American\n\nAssociation of Oral Maxillofacial Surgeons (AAOMS) or another\n\nprogram or examination as approved by the Board;\n\n2. A valid BLS certification;\n\n3. Employment and completion of a minimum of six (6) months of\n\ntraining under the direct supervision of a licensed oral\n\nmaxillofacial surgeon prior to starting DAANCE or another program or\n\nexamination as approved by the Board;\n\n4. Completion of a standardized course approved by the Board\n\nincluding a minimum of four (4) hours of didactic training that must\n\ninclude anatomy, intravenous access or phlebotomy, technique, risks\n\nand complications, and hands-on experience starting and maintaining\n\nintravenous lines on a human or simulator/manikin, and pharmacology;\n\nand\n\n5. Completion of an infection-control course as approved by the\n\nBoard.\n\nG. A dental assistant with an oral maxillofacial surgery\n\nexpanded duty permit shall complete eight (8) hours of continuing\n\neducation every two (2) years in classes approved by AAOMS that are\n\ncertified by the American Dental Association CERP program or another\n\nprogram approved by the Board. The continuing education requirement\n\nshall include at least one (1) hour on infection control.\n\nH. The Anesthesia Committee provided pursuant to Section 328.17\n\nof this title may make a recommendation to the Board for a dental\n\nassistant with an oral maxillofacial surgery expanded duty permit\n\nholding a temporary training permit to substitute training received\n\nfrom another state university, dental school or technical training\n\ninstitute or training acquired in a surgery center or hospital while\n\nworking under the authority of a licensed physician, to qualify as a\n\npartial substitute for the requirements to attain an oral\n\nmaxillofacial surgery expanded duty permit.\n\nI. A dental assistant with an oral maxillofacial surgery\n\nexpanded duty permit may only accept delegation from an oral and\n\nmaxillofacial surgeon, dentist, or pediatric dentist with a valid\n\ngeneral or parenteral anesthesia permit:\n\n1. Under direct supervision:\ncensed physician, to qualify as a\n\npartial substitute for the requirements to attain an oral\n\nmaxillofacial surgery expanded duty permit.\n\nI. A dental assistant with an oral maxillofacial surgery\n\nexpanded duty permit may only accept delegation from an oral and\n\nmaxillofacial surgeon, dentist, or pediatric dentist with a valid\n\ngeneral or parenteral anesthesia permit:\n\n1. Under direct supervision:\n\na. initiate and discontinue an intravenous line for a\n\npatient being prepared to receive intravenous\n\nmedications, sedation or general anesthesia, or\n\nb. draw up and prepare medications; and\n\n2. Under direct visual supervision:\n\na. follow instructions of the oral surgeon while acting\n\nas an accessory hand on behalf of the oral surgeon\n\nthat is administering the medication and actively\n\ntreating the patient. For the purposes of this\n\nsection, “administer” means to have the sole\n\nresponsibility for anesthesia care including\n\ndetermining medicines to be used and the dosage,\n\ntiming, route of delivery and administration of\n\nmedication and the assessment of the level of\n\nanesthesia and monitoring the physiological results of\n\nsuch care; provided, only an oral surgeon or dentist\n\npossessing a current general anesthesia permit may\n\nadminister or assess the level of sedation or general\n\nanesthesia and monitor the results of such care,\n\nb. follow instructions of the oral surgeon to adjust the\n\nrate of intravenous fluids to maintain or keep the\n\nline patent or open and adjust an electronic device to\n\nprovide medications such as an infusion pump, and\n\nc. assist the oral surgeon by reading, recording vital\n\nsigns of a patient receiving deep sedation or general\n\nanesthesia; provided, only an oral surgeon may assess\n\nthe level of sedation.\n\nJ. Only an oral surgeon shall be responsible to diagnose,\n\ntreat, monitor, determine and administer the selection of the drug,\n\ndosage, and timing of all anesthetic medications, and care of the\n\npatient through the perioperative period shall rest solely with the\n\nsupervising oral and maxillofacial surgeon.\n\nK.
