Any person, firm, corporation, partnership or other\n\nlegal entity who desires to operate a dental laboratory in this\n\nstate shall file with the Board of Dentistry, on a form prescribed\n\nby the Board, an application for a permit to operate a dental\n\nlaboratory and pay the fee established by the rules of the Board.\n\nThe application shall include the name and address of each person,\n\nfirm, corporation, partnership or other legal entity who owns an\n\ninterest in or will operate the dental laboratory. Upon receipt of\n\nthe application and fee, the Board shall determine the\n\nqualifications of the applicant and may grant a permit to the\n\napplicant to operate a dental laboratory.\n\n2. Except as provided in subsection C of this section, no\n\nperson, firm, corporation, partnership or other legal entity shall\n\noperate a dental laboratory in this state without having obtained a\n\npermit from the Board. The Board may inspect any dental laboratory\n\nprior to the issuance of any permit.\n\nB. Any change in ownership, operation or location of a dental\n\nlaboratory shall immediately be communicated to the Board, which\n\nshall endorse upon the permit, without further fee, the change in\n\nownership, operation or location.\n\nC. Nothing in the State Dental Act shall be construed to:\n\n1. Prohibit a dentist from owning or operating a private,\n\nnoncommercial dental laboratory in a dental office for the dentist's\n\nuse in the practice of dentistry;\n\n2. Require a dentist to obtain a permit from the Board for the\n\noperation of a dental laboratory in the office of the dentist unless\n\ndental laboratory technology is provided to persons other than the\n\ndentist at that location; or\n\n3. Require a dentist to issue a laboratory prescription for\n\ndental laboratory technology to be performed by an employee of, in\n\nthe office of, and for a patient of, the dentist.\n\nD. The dental laboratory shall make available to the\n\nprescribing dentist, Board, or agent or employee of the Board:\n\n1. A list of all materials in the composition of the final\n\nappliance;\n\n2. The location where the appliance was fabricated, including\n\nthe name, address, telephone number and Food and Drug Administration\n\nregistration number, if applicable, of the person or entity\n\nperforming the work; and\n\n3. A description of all disinfection methods used in the\n\nfabrication of the appliance.\n\nE. No permit shall be required for a licensed dentist in the\n\nState of Oklahoma, the licensed dentist's dental practice on-site\n\ndental lab, the licensed dentist's physical practice, or the\n\nlicensed dentist's CAD, CAM, 3-D or other technology used for\n\nfabricating dental prostheses including crowns, bridges and other\n\ndental restorations.
