The Board of Dentistry is authorized to charge the following\n\nfees for the purpose of implementing and enforcing the State Dental\n\nAct. The penalty and late fee shall be twice the amount of the\n\noriginal fee for license renewals. Notwithstanding any other\n\nprovisions of the State Dental Act, the fees established by the\n\nBoard shall be not less nor more than the range created by the\n\nfollowing schedule:\n\n1. LICENSE AND PERMIT APPLICATION FEES:\n\nMinimum Maximum\n\na. License by Examination\n\nDentist $200.00 $400.00\n\nDental Hygienist $100.00 $200.00\n\nb. License by Credentialing\n\nDentist $500.00 $1,000.00\n\nDental Hygienist $100.00 $200.00\n\nc. Dental Specialty License by\n\nExamination $300.00 $600.00\n\nd. Dental Specialty License by\n\nCredentialing $500.00 $1,000.00\n\ne. Faculty Permit\n\nDentist $100.00 $200.00\n\nDental Hygienist $50.00 $100.00\n\nf. Dental Student Intern Permit $50.00 $200.00\n\ng. Temporary License to Practice\n\nDental Hygiene $50.00 $100.00\n\nh. Dental Assistant Permit $50.00 $100.00\n\ni. Dental Assistant with\n\nExpanded Duty or Duties by\n\nCredential $100.00 $200.00\n\nj. Temporary License to Practice\n\nDentistry $75.00 $150.00\n\nk. Permit to Operate a Dental\n\nLaboratory – current Oklahoma\n\nlicensed dentist $20.00 $60.00\n\nl. General Anesthesia Permit\n\nDentist $100.00 $200.00\n\nm. List of the Name and Current\n\nMailing Address of all\n\nPersons who hold a License or\n\nPermit issued by the Board.\n\n(A request for a list shall\n\nbe submitted to the Board in\n\nwriting noting the specific\n\nproposed use of the list.) $25.00 $75.00\n\nh. Official State Dental License\n\nIdentification Card with\n\nPicture $25.00 $35.00\n\ni. Returned checks $25.00 $30.00\n\nB.
