Prescriptions received by other than written communication\n\nshall be promptly recorded in writing by the pharmacist. The record\n\nmade by the pharmacist shall constitute the original prescription to\n\nbe filled by the pharmacist.\n\nB. A filled prescription label shall include the name and\n\naddress of the pharmacy of origin, date of filling, name of patient,\n\nname of prescriber, directions for administration, and prescription\n\nnumber. The symptom or purpose for which the drug is prescribed may\n\nappear on the label if provided by the practitioner and requested by\n\nthe patient or the patient's authorized representative. If the\n\nsymptom or purpose for which a drug is prescribed is not provided by\n\nthe practitioner, the pharmacist may fill the prescription without\n\ncontacting the practitioner, patient, or patient's representative.\n\nFilled prescriptions issued for veterinarian drugs shall be labeled\n\naccording to rules promulgated by the Oklahoma State Board of\n\nVeterinary Medical Examiners. The label shall also include the\n\ntrade or generic name, prescribed quantity, and prescription\n\nstrength of the drug therein contained, except when otherwise\n\ndirected by the prescriber. This requirement shall not apply to\n\nprescriptions or medicines and drugs supplied or delivered directly\n\nto patients for consumption on the premises of any hospital or\n\nmental institution. This requirement shall not apply to dialysate\n\nsold, dispensed or delivered in their original, sealed packaging\n\nupon receipt of a prescriber's order.\n\nC.
