Ambulatory surgical centers;\n\n4. Health care facilities owned or operated by the state or\n\nfederal government;\n\n5. Skilled nursing facilities;\n\n6. Assisted living facilities;\n\n7. Prosthetic or orthotic practitioners;\n\n8. Health care practitioners who are licensed to practice\n\nhealth care in this state and who provide durable medical equipment\n\nwithin the scope of their health care practice;\n\n9. Manufacturers or wholesale distributors that do not sell or\n\nrent durable medical equipment directly to consumers;\n\n10. Suppliers of insulin infusion pumps and related supplies or\n\nservices; or\n\n11.
