The State Board of Licensure for Professional Engineers and\n\nSurveyors shall keep a record of its proceedings and of all\n\napplications for licensing, which record shall show:\n\n1. The name, date of birth and last-known mailing and email\n\naddress of each applicant;\n\n2. The date of application;\n\n3. The place of business of the applicant;\n\n4. The education, experience and other qualifications of the\n\napplicant;\n\n5. The type of examination required;\n\n6. Whether or not the applicant was rejected;\n\n7. Whether or not a license was granted;\n\n8. The date of the action of the Board;\n\n9. The board-approved area(s) of competence in a specific\n\ndiscipline(s) or branch(es) of engineering;\n\n10. A declaration under penalty of perjury from each applicant\n\nthat he or she will abide by the statutes and rules prescribed by\n\nthe Board, with the declaration becoming a part of his or her\n\napplication for licensing; and\n\n11. Such other information as may be deemed necessary by the\n\nBoard.\n\nB. The Board shall keep a record of all applications for a\n\ncertificate of authority, which shall show all of the following:\n\n1. The name, date of formation, and business address of each\n\napplicant;\n\n2. The date of application;\n\n3. The name, physical address, and license number of the\n\nmanaging agent;\n\n4. Whether or not the application was rejected;\n\n5. Whether or not a certificate of authority was granted;\n\n6. The date of the action by the Board;\n\n7. Services offered from each location;\n\n8. A declaration under penalty of perjury from an officer and\n\nmanaging agent, if the officer is not the managing agent, that the\n\napplicant will abide by the statutes and rules prescribed by the\n\nBoard, with the oath becoming a part of its application for a\n\ncertificate of authority; and\n\n9. Any other information deemed necessary by the Board.\n\nC. The record of the Board shall be prima facie evidence of the\n\nproceedings of the Board and a transcript thereof, duly certified by\n\nthe Secretary or Executive Director of the Board under seal, shall\n\nbe admissible as evidence with the same force and effect as if the\n\noriginal were produced.\n\nD. The Board shall submit, upon request from the Governor, a\n\nreport of its transactions of the preceding year, including a\n\ncomplete statement of the receipts and expenditures of the Board,\n\nattested by affidavits of its Chair and its Secretary.\n\nE.
