Application for a professional engineer, professional\n\nstructural engineer, or professional surveyor license, or\n\ncertification as an engineer intern or surveyor intern, shall be on\n\na form prescribed and furnished by the Board. It shall contain\n\nstatements made under oath, showing the applicant's education and a\n\ndetailed summary of technical and engineering or surveying\n\nexperience and shall include the names and complete mailing\n\naddresses of the references, none of whom may be members of the\n\nBoard or immediate family members of the applicant.\n\n2. The Board may accept the certified information contained in\n\na valid NCEES Record issued by the National Council of Examiners for\n\nEngineering and Surveying for professional engineer or professional\n\nsurveyor applicants in lieu of the same information that is required\n\non the form prescribed and furnished by the Board. All initial\n\napplicants for a license must submit an NCEES Record along with any\n\nadditional required forms to be considered for licensure.\n\nB. 1. The application fees shall be established by Board\n\nrules.\n\n2. The certification fee for a firm shall be established by\n\nBoard rules.\n\n3.
