The State Board of Medical Licensure and Supervision shall\n\ncreate such application forms as are necessary for the licensure of\n\napplicants to practice medicine and surgery in this state.\n\nB. No person shall be licensed to practice medicine and surgery\n\nin this state except upon a finding by the Board that such person\n\nhas fully complied with all applicable licensure requirements of\n\nthis act and has produced satisfactory evidence to the Board of the\n\nability of the applicant to practice medicine and surgery with\n\nreasonable skill and safety.\n\nC.
