An applicant to practice medicine and surgery in this state\n\nshall provide to the State Board of Medical Licensure and\n\nSupervision and attest to the following information and\n\ndocumentation in a manner required by the Board:\n\n1. The applicant’s full name and all aliases or other names\n\never used, current address, Social Security number, and date and\n\nplace of birth;\n\n2. A photograph of the applicant, taken within the previous\n\ntwelve (12) months;\n\n3. All documents and credentials required by the Board, or\n\nnotarized photocopies or other verification acceptable to the Board\n\nof such documents and credentials;\n\n4. A list of all jurisdictions, United States or foreign, in\n\nwhich the applicant is licensed or has applied for licensure to\n\npractice medicine and surgery or is authorized or has applied for\n\nauthorization to practice medicine and surgery;\n\n5. A list of all jurisdictions, United States or foreign, in\n\nwhich the applicant has been denied licensure or authorization to\n\npractice medicine and surgery or has voluntarily surrendered a\n\nlicense or an authorization to practice medicine and surgery;\n\n6. A list of all sanctions, judgments, awards, settlements, or\n\nconvictions against the applicant in any jurisdiction, United States\n\nor foreign, that would constitute grounds for disciplinary action\n\nunder the Oklahoma Allopathic Medical and Surgical Licensure and\n\nSupervision Act or the Board’s rules;\n\n7. A detailed educational history, including places,\n\ninstitutions, dates, and program descriptions, of all his or her\n\neducation, including all college, preprofessional, professional, and\n\nprofessional graduate education;\n\n8. A detailed chronological history from age eighteen (18)\n\nyears to the present, including employment, military service (United\n\nStates or foreign), and all professional degrees, licenses, or\n\ncertificates now or ever held; and\n\n9. Any other information or documentation specifically\n\nrequested by the Board that is related to the applicant’s ability to\n\npractice medicine and surgery.\n\nB. The applicant shall possess a valid degree of Doctor of\n\nMedicine from a medical college or school located in the United\n\nStates, its territories or possessions, or Canada that was approved\n\nby the Board or by a private nonprofit accrediting body approved by\n\nthe Board at the time the degree was conferred. The application\n\nshall be considered by the Board based upon the product and process\n\nof the medical education and training.\n\nC. The applicant shall have satisfactorily completed twelve\n\n(12) months of progressive postgraduate medical training as\n\npromulgated by rule by the Board. The Board shall consider as\n\nevidence of acceptability the sponsoring institution’s accreditation\n\nby the Accreditation Council for Graduate Medical Education (ACGME),\n\nthe American Society of Transplant Surgeons (ASTS), the Royal\n\nCollege of Physicians and Surgeons of Canada, the College of Family\n\nPhysicians of Canada, the Royal College of Surgeons of Edinburgh,\n\nthe Royal College of Surgeons of England, the Royal College of\n\nPhysicians and Surgeons of Glasgow, or the Royal College of Surgeons\n\nin Ireland.\n\nD. The applicant shall submit a history from the administration\n\nof the medical school from which the applicant graduated of any\n\nsuspension, probation, or disciplinary action taken against the\n\napplicant while a student at that institution.\n\nE. The applicant shall have passed medical licensing\n\nexamination(s) consistent with policies and procedures regarding the\n\nnumber of medical licensure exam attempts as established by the\n\nUnited States Medical Licensing Examination program unless granted\n\nan exception by the Board.\n\nF. The applicant shall have demonstrated a familiarity with all\n\nappropriate statutes and rules and regulations of this state and the\n\nfederal government relating to the practice of medicine and surgery.\nicies and procedures regarding the\n\nnumber of medical licensure exam attempts as established by the\n\nUnited States Medical Licensing Examination program unless granted\n\nan exception by the Board.\n\nF. The applicant shall have demonstrated a familiarity with all\n\nappropriate statutes and rules and regulations of this state and the\n\nfederal government relating to the practice of medicine and surgery.\n\nG. The applicant shall be physically, mentally, professionally,\n\nand morally capable of practicing medicine and surgery in a manner\n\nreasonably acceptable to the Board and in accordance with federal\n\nlaw and shall be required to submit to a physical, mental, or\n\nprofessional competency examination or a drug dependency evaluation\n\nif deemed necessary by the Board.\n\nH. The applicant shall not have committed or been found guilty\n\nby a competent authority, United States or foreign, of any conduct\n\nthat would constitute grounds for disciplinary action under the\n\nOklahoma Allopathic Medical and Surgical Licensure and Supervision\n\nAct or rules of the Board. The Board may modify this restriction\n\nfor cause.\n\nI. Upon request by the Board, the applicant shall make a\n\npersonal appearance before the Board or a representative thereof for\n\ninterview, examination, or review of credentials. At the discretion\n\nof the Board, the applicant shall be required to present his or her\n\noriginal medical education credentials for inspection during the\n\npersonal appearance.\n\nJ. The applicant shall be held responsible for verifying to the\n\nsatisfaction of the Board the identity of the applicant and the\n\nvalidity of all credentials required for his or her medical\n\nlicensure. The Board may review and verify medical credentials and\n\nscreen applicant records through recognized national physician\n\ninformation services.\n\nK. The applicant shall have paid all fees and completed and\n\nattested to the accuracy of all application and information forms\n\nrequired by the Board.\n\nL. Grounds for the denial of a license shall include:\n\n1. Use of false or fraudulent information by an applicant;\n\n2. Suspension or revocation of a license in another state\n\nunless the license has been reinstated in that state;\n\n3. Refusal of licensure in another state other than for\n\nexamination failure; and\n\n4. Multiple examination failures.\n\nM.
