Stat. tit. 59, § 59-642","heading":"Failure to comply with license renewal requirements -","body":"Cancellation of license - Reinstatement - Inactive status prohibited\n\n- Voluntary cancellation of license.\n\nA. If any licensee shall fail to comply with the requirements\n\nof Section 641 of this title or this section and such license is\n\nallowed to lapse, the licensee shall, upon order of the State Board\n\nof Osteopathic Examiners, forfeit the right to practice osteopathic\n\nmedicine in this state and the license and certificate shall be\n\ncanceled; provided, however, that the Board may reinstate such\n\nperson upon the payment of all fees due, plus a penalty fee in the\n\namount fixed by the State Board of Osteopathic Examiners not to\n\nexceed twice the amount of the license renewal fees as determined by\n\nthe Board and upon the presentation of satisfactory evidence of the\n\nattendance at an educational program as provided for in Sections\n\n637, 637.1, and 641 of this title and such other proof of\n\nprofessional conduct as prescribed by rule. The State Board of\n\nOsteopathic Examiners shall not place the license of any person\n\nauthorized to practice osteopathic medicine in this state on\n\ninactive status.\n\nB. Any licensee whose renewal application is rejected by the\n\nBoard shall have the right to appeal from such action to the\n\ndistrict court of the county of residence. If the licensee does not\n\nreside or practice in this state, appeal shall be to the Oklahoma\n\nCounty District Court.\n\nC. Licensees who retire from such practice or desire to request\n\ncancellation of their license shall file with the State Board of\n\nOsteopathic Examiners an affidavit, on a form to be furnished by the\n\nBoard, which states the date of retirement and such other facts to\n\nverify the retirement or other reasons for cancellation as the Board\n\nmay deem necessary and the license shall be canceled.
