A contact lens prescription shall include the following:\n\n1. The ophthalmic information necessary to accurately fabricate\n\nor dispense the lenses, including the lens manufacturer, lens\n\nseries/brand name and the lens material, if applicable;\n\n2. Power and base curve;\n\n3. Name, license number, telephone number and, for written\n\norders, the signature of the prescribing optometrist;\n\n4. Patient's name and address, expiration date of the\n\nprescription and number of refills or lenses permitted; and\n\n5. The date of issuance.\n\nB. A contact lens prescription may also include the diameter,\n\naxis, add power, cylinder, peripheral curve, optical zone and center\n\nthickness.\n\nC. A prescription for visual aid glasses shall include the\n\nfollowing:\n\n1. The name, license number, telephone number and, for written\n\norders, the signature of the prescribing optometrist;\n\n2. The patient's name;\n\n3. The date of issuance; and\n\n4. The value of all parameters the Oklahoma-licensed\n\noptometrist has deemed necessary to dispense corrective lenses\n\nappropriate for a patient.\n\nD.
