Stat. tit. 59, § 59-887.7","heading":"Application for licenses - Fees","body":"Any person intending to practice as a physical therapist or\n\nphysical therapist assistant in this state shall apply to the Board\n\nin writing. Such application shall be on a form and in a manner\n\nprescribed by the Board and shall request such information from the\n\napplicant as will indicate to the Board the applicant's\n\nqualifications to take the required examination or otherwise comply\n\nwith the provisions of the Physical Therapy Practice Act.
