Except as provided in subsection B of this section, on every\n\ncontract entered into by any county, school district, technology\n\ncenter school district or political subdivision of the state for an\n\narchitect, contractor, engineer or supplier of construction\n\nmaterials of Twenty-five Thousand Dollars ($25,000.00) or more,\n\nshall be the following signed statement:\n\nSTATE OF OKLAHOMA)\n\n) ss.\n\nCOUNTY OF)\n\nThe undersigned (architect, contractor, supplier or engineer),\n\nof lawful age, being first duly sworn, on oath says that this\n\ncontract is true and correct. Affiant further states that the\n\n(work, services or materials) will be (completed or supplied) in\n\naccordance with the plans, specifications, orders or requests\n\nfurnished the affiant. Affiant further states that (s)he has made\n\nno payment directly or indirectly to any elected official, officer\n\nor employee of the State of Oklahoma, any county or local\n\nsubdivision of the state, of money or any other thing of value to\n\nobtain or procure the contract or purchase order.\n\n_____________________________________\n\n(Contractor, architect, supplier or\n\nengineer)\n\nAttested to before me this ___ day of _______, 20___.\n\nB.
