The State Commissioner of Health shall promulgate rules\n\nnecessary to develop a uniform application which shall be used in\n\nthe credentialing process of health care providers. The State\n\nDepartment of Health shall develop such application form for:\n\n1. Initial privileges or membership in a hospital or other\n\nentity requiring credentials verification, except for a contracted\n\nentity as defined by Section 4002.2 of Title 56 of the Oklahoma\n\nStatutes; and\n\n2. Recredentialing or reappointment in a hospital or other\n\nentity requiring credentials verification, except for a contracted\n\nentity as defined by Section 4002.2 of Title 56 of the Oklahoma\n\nStatutes.\n\nB.
