Unless otherwise provided in the Oklahoma Public Health\n\nCode:\n\n1. Any person who willfully fails or refuses to comply with, or\n\nviolates, a lawful order of the State Board of Health or the State\n\nCommissioner of Health, or his duly authorized representative, or of\n\na local health officer, or who violates the terms and conditions of\n\na quarantine or embargo, shall, upon conviction, be guilty of a\n\nmisdemeanor, and upon conviction thereof may be punished by a fine\n\nof not to exceed One Hundred Dollars ($100.00), or by imprisonment\n\nin the county jail for not more than thirty (30) days, or by both\n\nsuch fine and imprisonment;\n\n2. Any person who fails or refuses to make or file a report, or\n\nto file a certificate, or to keep a record, that is required by the\n\nprovisions of this Code, or by rules of the State Board of Health,\n\nor the State Commissioner of Health, or who gives false information\n\nin or for such report, certificate or record, shall, upon\n\nconviction, be guilty of a misdemeanor, and upon conviction thereof\n\nmay be punished by a fine of not more than Two Hundred Dollars\n\n($200.00);\n\n3. Any person who gives false information in an application for\n\na license or permit, or to the Commissioner or a local health\n\nofficer, shall, upon conviction, be guilty of a misdemeanor, and\n\nupon conviction thereof may be punished by a fine of not more than\n\nTwo Hundred Dollars ($200.00);\n\n4. Any person who does any act for which a license or permit is\n\nrequired by the provisions of this Code, and who is not at the time\n\nthe holder of such a license or permit, shall, upon conviction, be\n\nguilty of a misdemeanor, and upon conviction thereof may be punished\n\nby a fine of not more than Five Hundred Dollars ($500.00), or by\n\nimprisonment in the county jail for not more than six (6) months, or\n\nby both such fine and imprisonment; and\n\n5. Any person who does any act that is made unlawful or a\n\nmisdemeanor by the provisions of this Code, or who violates any of\n\nthe other provisions of this Code, or any standard, rule or\n\nregulation authorized by this Code, shall, upon conviction, be\n\nguilty of a misdemeanor, and upon conviction thereof may be punished\n\nby a fine of not more than Two Hundred Dollars ($200.00), or by\n\nimprisonment in the county jail for not more than thirty (30) days,\n\nor by both such fine and imprisonment.\n\nB. 1. Notwithstanding the penalties provided for in this\n\nsection, district courts may also grant injunctive relief to prevent\n\na violation of, or to compel a compliance with, any of the\n\nprovisions of this Code or any rule or order issued pursuant to this\n\nCode.\n\n2. Any action for injunctive relief to redress or restrain a\n\nviolation by any person of any provision of this Code, any rule or\n\norder issued pursuant to this Code, or recovery of any\n\nadministrative or civil penalty assessed pursuant to Section 1-\n\n1701.1A of this title may be filed and prosecuted by:\n\na. the district attorney in the appropriate district\n\ncourt of the State of Oklahoma, or\n\nb. the Department on behalf of the State of Oklahoma in\n\nthe appropriate district court of the State of\n\nOklahoma, or as otherwise authorized by law.\n\n3.
